Patients concerned that they may have coronavirus or may have come into contact with someone who has please DO NOT come into surgery.

As of today any booked or future appointments in line with government guidance we will be operating a telephone consultation system.
Due to advice from Public Health England we are temporarily stopping our 'drop in' baby clinics. Patients wishing to be seen at baby clinic must telephone the surgery to be booked into an appointment slot. This is to minimise risk to both patients and staff. This will take effect immediately.
Church View Surgery Patients
Due to the current situation with Corona Virus, Dr Taylor and partners are centralising care at the Shaftesbury site from Friday 20th March. Contact numbers will remain the same. Our address at Shaftesbury Medical Centre is 1st Floor, East Leeds Health Centre, 78 Osmondthorpe Lane, Leeds LS9 9EF.
You will not be able to access the Church View site during this time but we will continue to provide care from Shaftesbury Medical Centre.

Repeat Medication
Do not come to surgery to order medicines. Only order your usual quantity of regular medicines when due.
We will issue prescriptions ONLY when due or when a qualified member of staff tells us it is clinically essential. We will not allow stockpiling of medicines.
All prescriptions are now sent electronically to pharmacies. If this is new to you, we will contact you.
To order your medicines:
1. Sign up & use the NHS app www.nhs.uk/nhsapp
2. Order online using https://systmonline.tpp-uk.com
3. Email prescriptions.shaftesbury@nhs.net
4. Call your usual pharmacy. They will contact us with your request. Please be patient as they respond to huge increases in requests.

Help and Support for Domestic Violence
For anyone who feels they are at risk of domestic violence there is help and support available during the Coronavirus pandemic. You can still:
Call the police on 999 in an emergency (press 55 after the emergency number if you're in danger and not able to speak for help).
Find accommodation from Leeds Housing Options or Leeds Domestic Violence Service if you are a victim of domestic abuse and need to escape.
Find a safe place to stay with social distancing in place to protect you.

Complaints Process
Due to the ongoing COVID19 pandemic, we are currently pausing our complaints process, in line with national guidance.
You can still raise concerns/ make a complaint. We will review each complaint but will only respond to those that pose a significant threat to patient safety. All other complaints will be dealt with when normal services resume.

The practice has been long established, dating back to 1905. There are two sites
Shaftesbury Medical Centre
The Shaftesbury is a newly purpose built centre opened in 2008. It mainly looks after patients in Leeds 9 and 14.
There is car parking for patients on site at the Shaftesbury Medical Centre. The main reception and consulting rooms are on the first floor and there is lift and stair access.
There is a fully equipped operating theatre at the Shaftesbury where minor surgery is performed.
Opening Hours:
Monday 8.15am – 7.30pm
Tuesday 7.00am to 6pm
Wednesday, Thursday and Friday 8.15am – 6.00pm
Appointments are also available at Church View Surgery for all our patients on alternate Saturdays 8.00am to 10.30am
Church View Surgery
Church View Surgery is located on the second floor of a new purpose built medical centre. We are in the centre of Crossgates and easily accessible on foot from the Crossgates Shopping Centre.
There is patient parking, disabled access and a lift. The surgery has patients in Leeds 14 and 15.
Opening Hours:
Monday to Friday 8.15am to 6.00pm
Alternate Saturdays 8.00am to 10.30am
Appointments are also available for all our patients at Shaftesbury Medical Centre on Tuesdays from 7.00am.
Improving access to general practice: early morning, evening and weekend appointments now available.
You can now see a GP or other healthcare professional during:
- Weekday evenings between 6.30pm and 8pm
- Saturday 8.00am - 1.45pm
- Sunday 8.00am - 11.45am

Both surgeries have disabled access and are no smoking buildings.
