The practice has been long established, dating back to 1905. There are two sites
Shaftesbury Medical Centre
The Shaftesbury is a newly purpose built centre opened in 2008. It mainly looks after patients in Leeds 9 and 14.
There is car parking for patients on site at the Shaftesbury Medical Centre. The main reception and consulting rooms are on the first floor and there is lift and stair access.
There is a fully equipped operating theatre at the Shaftesbury where minor surgery is performed.
Tuesday and Friday 7.00am to 6.00pm
Monday 8.15am to 6.00pm
Wednesday and Thursday 8.15am to 6.00pm
Appointments are also available for working patients on alternate Saturdays at Church View Surgery from 8.30-1030am.
Church View Surgery
Church View Surgery is located on the second floor of a new purpose built medical centre. We are in the centre of Crossgates and easily accessible on foot from the Crossgates Shopping Centre.
There is patient parking, disabled access and a lift. The surgery has patients in Leeds 14 and 15.
Monday to Friday 8.15am to 6.00pm
Alternate Saturdays 8.30am to 10.30am
Appointments are also available for working patients at Shaftesbury Medical Centre on Tuesdays and Fridays from 7.00am.
Both surgeries have disabled access and are no smoking buildings.

Tuesday and Wednesday Afternoons - On Wednesday afternoons the surgery is open for patients with pre-booked appointments, to collect prescriptions etc. We do not have a doctor on call and phone calls will transfer to the out of hours treatment centre.
The surgery is closed for training purposes in the afternoon of – Tuesday 2th April 2017, 16th May 2017, 13th June 2017, 11th July 2017, 12th Sept 2017, 10th Oct 2017, 7th Nov 2017, 16th Jan 2018, 6th Feb 2018, 13th March 2018. If you need urgent medical attention when the surgery is closed please telephone the normal surgery number and your call will be transferred to the out of hours treatment centre.
